Mental Health Services
Person-centered behavioral health support promoting emotional wellness, recovery, improved daily functioning, and community participation.
1915(i) Individual & Transitional Support (ITS)
Skill-building for eligible individuals with serious mental health conditions or severe substance use disorders, emphasizing housing stability, budgeting, daily living, and community integration.
1915(i) Community Living & Supports (CLS)
Individualized assistance and skill development for eligible people with intellectual/developmental disabilities or traumatic brain injuries, supporting independence and meaningful community participation.
Home Care Services
Compassionate nonmedical assistance with activities of daily living and household routines to support dignity, safety, and comfort at home.
